Applications may ONLY be made for students enrolled in primary or secondary schools in the school year 2024/25.
Separate applications must be made for EACH child.
Submission of this application form DOES NOT guarantee receipt of a school supply & book grant.
Applicants are asked to provide truthful and accurate information. Incorrect and untrue information will result in disqualification.

REQUIRED DOCUMENTS

Before you start please ensure you have scanned copies of the following documents. All documents must be uploaded as a PDF document no larger than 12MB in size.

Verification Letter from the School.

Copy of Student's Birth Certificate
Medical evidence of student's disability (if applicable)

One (1) form of ID (National Identification Card / Passport / Driver's Permit)
Utility Bill (If the utility bill is not in your name, an Authorization letter will be required)
Job Letter / Pay Slip no older than 3 months, stamped by the employer; OR
Two (2) Letters of Recommendation for self / unemployed persons. (Must be Stamped)
Receipt for government grants currently received.
Bank Slip (To be requested for at the bank. The slip will contain Account Holder's Name, Branch and Account Number. This is NOT a bank statement)

Applicants are responsible for ensuring that the Bank Accounts provided remain active throughout 2025 to facilitate depositing of funds by the MOE. The MOE will not assume responsibility for funds sent to accounts that are dormant or inactive.
